For community vaccination events, advance registration is available but not required. To register in advance, please follow the instructions below. Advance registration will save you time once you arrive at the vaccination event.
There is no cost to receive the vaccine. Please bring your photo ID. If you are receiving a 2nd dose or booster bring your vaccine card.
Community Vaccination Event On-Line Registration
You DO NOT need to be a patient of MEHOP to receive the COVID-19 vaccination or booster from one of our locations or mobile COVID unit.
Persons 18 and older are eligible to receive a Moderna COVID-19 vaccine.
Persons 12 and older are eligible to receive a Pfizer COVID-19 vaccine. (MEHOP Does Not Currently Have Pfizer)
You are eligible for a booster 6 months after your second dose of Pfizer or Moderna . If you received the J&J Janssen, you are eligible 2 months after your one dose. It has been approved to receive a different booster COVID vaccine from the original vaccine you received.

- Please print and complete the COVID-19 Questionnaire and Consent for Vaccine Reporting form and bring to your appointment along with photo ID.
You will see a confirmation that you scheduled your appointment and will also receive an email confirmation. There could be a delay receiving your email confirmation, please allow some time for it to be received.
