New Patients

Being a Patient at MEHOP

New patients will need to fill out new patient forms. These forms are located on our website, simply click which services you are interested in and you will find the new patient forms located on that page. You may complete and print these from our website or request them at MEHOP. Forms are available in English and Spanish. If you need assistance, MEHOP staff will be available to assist you. Please remember to bring a list of all your current medications to your appointment.

Patients with Insurance

  • MEHOP Health Center is an in-network provider for Medicare, Medicaid (including CHIPS, Star and Star Plus) and many Private Insurance carriers. If a patient has insurance, their carrier will be billed according to services provided. Applicable co-pays and deductibles will apply. Benefits will be verified, so please remember to bring your insurance card to all appointments.

Patients without insurance or have limited insurance benefits

  • If you are uninsured or have limited insurance benefits, we receive grant funding for specific programs and offer discounts (sliding fees) for services based on household size and income, if eligible, to assist with the cost of health care services.
  • Please schedule an appointment with our Eligibility Department prior to scheduling an appointment with one of our providers to establish placement through grant funding or on our sliding fee scale, which is based on household size and income.
  • During your appointment with eligibility, we will require the following current information. The appointment with eligibility takes approximately 15-20 minutes. Once established, eligibility is valid for 6 months and will need to be updated there after.
      1. Proof of income for household (examples below)
        • Current Tax Return, 3 Pay Stubs, Unemployment verification, Social Security Award Letter, SNAP verification, etc.
      2. Utility bill for address verification
      3. Verification of those living in household
      4. Insurance Card ( if you have insurance)

Fees for Services

Services at MEHOP are not free. If a patient has insurance, their carrier will be billed according to services provided and all co-pays and deductibles will apply. If a patient is uninsured, they may ask when scheduling their first appointment what the general associated fees will be. If additional services are required, fees will be discussed with patients during office visit. Payment is expected at time of service.
