MEHOP is a recognized Level 3 Patient-Centered Medical Home (PCMH). PCMH is a care delivery model whereby patient treatment is coordinated through their primary care physician to ensure they receive the necessary care when and where they need it, in a manner they can understand.
The objective is to have a centralized setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.
Our physicians and nurse practitioners spend time getting to know their patients and provide experienced routine and acute care as well as the management, prevention, and treatment of chronic health conditions, such as diabetes. Each patient has access to quality medical care that is personalized to meet their individual healthcare needs.

MEHOP accepts all patients. We are an in-network provider for most private insurance carriers, Medicare and Medicaid.
If you are uninsured or have limited insurance benefits, we offer discounts (sliding fees) for services based on household size and income if eligible. Health Center programs assure that no patient will be denied or unable to access health care services due to an individual's inability to pay for services and assure that any fees or payments required by the center will be reduced to enable the center to fulfill this goal.
If you are an existing patient and would like to schedule an appointment, request medical records, ask a provider a question or check on your prescription you may access patient portal by emailing us at patientportal@MEHOP.org. Patient Portal is the new, easy way to access your medical information in a secure web format. Simply send us an email requesting access to your information and we will send you a unique pin number for your account.
If you have a medical emergency, please call 911. If you have a medical question/concern that arises after normal business hours and is unable to wait until the following business day, you may contact our after hours on-call personnel at 979-318-9853. If you need to schedule/change an appointment you will need to call during normal business hours.
